What causes List index to go out of bounds? How to avoid it?

Briefly, input file is H.264 TS, output file is mpeg2.  Input file size is 340KB trimmed down to avoid the disk space too small problem.

Windows XP, Intel Core 2- 1.6GH, 1.25G Ram , 2.5G disk sp on C, output file goes to D drive with >100G
